About MY Vitality Therapy

We strive to prevent hospitalizations and premature placement of older adults into skilled nursing and long term care facilities due to chronic disease or disability. It is our goal to deliver quality habilitative and rehabilitative services to older adults and their care partners in their homes in order to facilitate their safety, independence, and engagement in meaningful and necessary activities of daily living. We understand the stresses of caregiving and strive to assist care partners in maintaining their ability to care for loved ones safely at home.

About Michelle Yentsch, MS, OTR/L, FMCHC, CPAM

Michelle graduated from the University of Virginia with a BA in Psychology and spent the first part of her career working within Johns Hopkins Outpatient NeuroRehabilitation Program, which spawned her passion for mind-body health and rehabilitation. She went on to pursue an MS in Occupational Therapy from Towson University and has since gained experience in subacute rehab, long-term care, and outpatient settings.

Over the past few years, Michelle has completed additional training to become certified as a functional medicine health coach, certified in the administration of physical agent modalities in Maryland, and trained to deliver the Skills2Care® program for care partners of people with dementia. She also brings forth over ten years of experience co-facilitating a support group for families of individuals affected by young onset dementia.

Michelle is committed to a holistic and evidence-based treatment process that involves partnership with her clients. Michelle founded MY Vitality Therapy with a vision to provide exceptional care and support for individuals and their families navigating the challenges of aging at home. The name MY Vitality Therapy is derived from Michelle’s initials and one of her primary values: vitality, or the continuation of a purposeful and meaningful existence

Our services encompass a holistic, collaborative, and empathetic process. By offering in-home therapy, we can better understand and treat clients in their immediate environment, making it easier and safer for them to age at home.

What is the process to get started?

  1. Book free discovery call to determine if outpatient occupational therapy is right for you
  2. Submit insurance information and date of birth to allow MY Vitality Therapy to verify your eligibility
  3. Obtain referral for services from PCP
  4. Once referral is received, MY Vitality Therapy will reach out to schedule the initial evaluation
